Sponsor Form: Godparent-Sponsor Form

The Sponsor form must be completed by the Sponsor and the Sponsor’s Pastor. It must include the Pastor’s signature and the seal of that church at the bottom of the form.

Saint Name Form: Saint Name Form

The Saint Name Form is filled out by the child receiving the Sacrament of Confirmation with assistance from the parents if needed.
